What Is Home Health Care?

When a patient is recovering from a procedure or illness and requires ongoing treatment, they may have the option of home care. While remaining at home, Continuum provides a skilled medical team of therapists, nurses, and nurse aides designed to fit the individual needs of the patient.

Continuum Home Health Care focuses on recovery, rehabilitation and wellness management. Each care plan brings a different course of treatment with the same compassionate care and education on how to maintain progress that one would normally receive in an inpatient setting.

Our Home Health Approach

Our home-based treatment includes a personally tailored care plan developed by our team and your physician to support a recovery that will allow you to return to your previous lifestyle. Additionally, we will coordinate with equipment providers, specialty physicians, and pharmacies giving you the freedom to concentrate on your recovery and enjoy time at home.

Continuum Home Health Care provides patient-centered, quality care focused on rehabilitation to achieve both personal and physician-set goals. Patients and families can expect constant communication and support from a trusted, familiar healthcare professional when they choose Continuum.

Our Team

Certified Nursing Assistants (CNAs) provide assistance with personal hygiene, feeding and other activities of daily living. They are responsible for helping with medications, medical devices and checking vitals and relaying this information to the RN or case manager. They also offer a friendly and familiar face that allows for positive patient and family interaction.

There are multiple therapy types offered. Physical, occupational and speech therapists are available to help patients maximize their current functional capabilities and achieve goals set by the rest of the medical team

Our Medical Social Workers provide social and emotional support for the patient and family. They coordinate community resources and provide education as needed.

The Registered Nurse (RN) and Case Manager evaluate the patient and develop the plan of care with input from the patient and family, as well as other team members. The Case Manager visits the patient frequently to ensure the patient’s comfort and provide treatment.
